New Patients (Your First Visit):

When you walk in to the office, please Check In on the Computer located at the front desk.

Fill out 'I'm New Here' information screen on the check in Kiosk, as well as fill out and sign all pages on the Clipboard.

Allow at least 30-45 minutes for your first appointment. This will include Check-In, initial client paperwork, wait time, consultation, exam & adjustment, and check out procedures.

Full payment is due at the time of service as this office does not bill any medical insurance. See 'FEES' page for details.

Follow-Up Visits:

Although the adjustment with Dr. Delacruz may only take a few minutes, there may be a wait time depending on the time of day and the amount of clients in the office.
